Our Hospice Care Program Includes:

Nursing Registered

Nurses perform comprehensive assessments, assist with pain and symptom management and conduct thorough education to patients and their families. Nurses coordinate care with various members of the interdisciplinary team (IDT) and outside vendors for overall management of patient's condition.

Physician Services

The Hospice Medical Director provides direction to the hospice team and partners with the patient's Attending Physician in order to ensure coordinated and collaborative care.

​Social Services

Master's Level Social Worker provides psychologic support, family counseling, community resources and financial assistance.

Hospice Aides

Hospice Aides provide personal care and assistance with environmental hygiene through intermittent visits to patient's home.

Medication

​Thorough education on pain and symptom management medications from a qualified and licensed Registered Nurse.

Volunteers especially trained to provide companionship and support to patients and their loved ones during very difficult times.

​Medical Supplies

Hospice covered medical necessities including incontinence care supplies, oral care supplies, wound care supplies and personal hygiene supplies.

Durable Medical Equipment

Medical Equipment necessary to ensure comfort and assist with physical impairments including (but not limited to) hospital bed, oxygen concentrator, walker and wheelchair.

Bereavement & Grief Support

Headed by the Hospice Chaplain, bereavement team and volunteers provide emotional and spiritual support to patient's families and loved ones.

Physical, Occupational & Speech Therapists

Specialty modalities provide assistance to manage pain and other symptoms.

Spiritual Care & Counseling

Support services focused on emotional, mental and spiritual care of patients and their families. Hospice Chaplain assists with resources for end-of-life issues. Hospice Chaplain also available for crisis intervention at the time of patient's death and able to assist with memorial services.
 

Grief Support for those who are suffering from the loss of a loved one are offered on the last Wednesday of every month from 6 to 7 pm at Angels Hospice, Castro Valley.
