Admission Criteria
Terminal, life-limiting diagnosis with a prognosis of 6 months or less provided that the disease follows its normal course.
Approval of the hospice Medical Director and the Attending Physician, who may continue to supervise the care of the patient.
Patient (or representative) consent to enroll in hospice program.
Able caregiver to provide for patient's daily needs who will receive guidance from hospice team.
We can assist in finding a caregiver for those without one
Hospice care is not just for cancer patients
Although cancer patients may be eligible to receive hospice services, Angels Hospice also provides care for patients with:
HIV/AIDS
COPD
End stage Alzheimer's disease
End stage Parkinson's disease
End stage respiratory disease
End stage cardiac disease
End stage liver or kidney disease
Multiple Sclerosis (MS) or Amyotrophic Lateral Sclerosis (ALS)
